Assistance
Homeless to housing program
Hennepin County residents who are experiencing homelessness can receive housing case management assistance through the Homeless to Housing program. We have solutions to homelessness, and have the tools to help people find and sustain housing.
Services
Homeless to Housing teams can help single adults and families staying in emergency shelter or living unsheltered with the following process as they work on a plan to get into stable and permanent housing:
- Referral
- Intake
- Goal planning
- Barrier busting
- Housing identification and move in
- Stabilization and referral to long-term support
Eligibility
You are eligible for the Homeless to Housing program if you are:
- Staying in a homeless shelter, or sleeping outside or in a vehicle
- 18 years old or older
- Not already working with a housing case manager

What to expect from the process
Once you are added to the interest list, and as caseload openings become available, the team will follow up, confirm eligibility, and then start working with you.
